Incident Reporting
For the reporting of injuries, illnesses, exposure to hazardous substances and near-misses.
Incident and near miss reporting, investigation, and tracking provide the Environmental Health and Safety Department (EHS) with an opportunity to glean information from the unintended outcomes and use that information to help prevent future incidents. Additionally, the university is required to assure correct, adequate and prompt reporting of all incidents to necessary agencies in a timely manner. Incident information is also needed for effective worker’s compensation implementation. Therefore, each member of the university community has the responsibility to report incidents and near misses promptly and serve as an active participant in the inquiry into the cause of the incident.
Injury/illness/exposure requiring non-emergency medical treatment
- The employee immediately reports the incident to their supervisor
- The supervisor:
- Responds to the incident and secures the scene
- Provides the employee with a printed Workers’ Compensation Panel Physicians Form and returns the completed form to Human Resources as soon as it is received HRBENEFITS@cnu.edu
- Conducts an incident investigation (contact the EHS Department if guidance is needed)
- Completes and submits the Employee Incident Report Form within 24 hours
The employee seeks medical care from the selected panel physician and provides all medical notes to Human Resources HRBENEFITS@cnu.edu
Injury/illness/exposure requiring emergency medical treatment
- Employee or first responder calls CNU Police (757) 594-7777 for a life-threatening medical emergency.
- If an incident occurs that requires admission to the hospital and/or involves amputation, loss of an eye, or a fatality, notify the EHS Department immediately at (757) 594-7280. Please leave a voice mail as needed and after hours. The following information should be provided when reporting a serious injury/medical emergency:
- Name of injured person
- Location of incident
- Date and time of incident
- What happened (be as specific as possible)?
- Were there any fatalities?
- Was anyone hospitalized?
- Name, email, and phone number of the person reporting the incident
- The supervisor should notify Human Resources as soon as possible after the employee obtains medical assistance.
- The supervisor must complete the Employee Incident Report Form as soon as possible, preferably within four (4) hours of the incident
Employees are required to participate in an accident investigation either before or after medical treatment, depending upon the urgency of the treatment.

Injuries to students on university-owned or leased property
The General Incident Report Form should be completed by the individual to whom the incident was reported within 24 hours of the incident. For incidents occurring in laboratories, please also submit the Student Laboratory Incident Report Form. Incidents involving student employees performing work-related activities should be reported and investigated according to the employee process.For additional assistance please reach out to the EHS Department.
When contractors are injured on university-owned property, the appropriate contract administrator should complete the General Incident Report Form within 24 hours of the incident.
When visitors are injured on university-owned property, the Christopher Newport employee or other appropriate affiliates who were witness to the incident should complete the General Incident Report Form within 24 hours of the incident.
Employees should request assistance from their supervisors when reporting incidents involving contractors or visitors.

Near misses are incidents in which no property was damaged and no personal injury was sustained, but where, given a slight shift in time or position, damage or injury easily could have occurred. Please use the Near Miss Report Form to report these circumstances to the EHS Department in a timely manner. The information will be used to help prevent injury and illness.
